When globe tenting becomes urgent
Severe tenting can indicate dangerous increases in orbital pressure. In acute proptosis, marked narrowing of the posterior globe angle may warrant urgent decompression to protect vision.
Red flags to escalate
- Acute proptosis with rapidly worsening pain or swelling
- Decreased visual acuity, afferent pupillary defect, color vision loss
- Posterior globe angle <120° on CT (when measurable)
- Orbital apex crowding / significant optic nerve tension

- Confirm the presence and severity of tenting
- Identify the underlying cause (abscess, hemorrhage, vascular lesion)
- Support time-critical decision making with the clinical team
